What proof of a structured diet is required for lap band surgery?

I have Carefirst BlueCross BlueShield. I have been a registered user of Weight Watchers online for 4 years. Will that work? How much proof do they require? Any other ideas of proof I could show them without doing an actual supervised diet from my doctor? Thanks in advance.
